Super Speciality Hospital performs rare procedure to treat brain aneurysm

When everywhere the discussion is going on regarding coronavirus cases, one different case had come at Super Speciality Hospital. The doctors at the hospital treated this case by performing a rare and difficult procedure and gave a new lease of life to a 45-year-old woman. The 45-year-old woman, Suman Bhade was admitted in a private hospital at Akola with history of sudden onset severe headache followed by unconsciousness. Patient was primarily managed at Akola and was investigated on the basis of her CT brain. The CT pointed out bleeding inside brain (IVH) because of rupture of aneurysm (balloon) at very rare and complex site in brain stem.
 
The place near brain stem is very risky and difficult for medicos to conduct procedure. Patient was referred to Super Speciality Hospital at Nagpur for further expertise management. Explaining about brain aneurysm, Dr Pramod Giri, noted neurosurgeon and professor at Super Speciality said, “A brain aneurysm is a bulge or ballooning in a blood vessel in the brain. It often looks like a berry hanging on a stem. A brain aneurysm can leak or rupture, causing bleeding into the brain (haemorrhagic stroke). Most often a ruptured brain aneurysm occurs in the space between the brain and the thin tissues covering the brain. This type of hemorrhagic stroke is called a subarachnoid haemorrhage.
 
A ruptured aneurysm quickly becomes life-threatening and requires prompt medical treatment.” Initially, the patient was admitted in Government Medical College and Hospital (GMCH), for initial workup. During her check-up an opinion regarding neurosurgery was taken. Neurosurgeon advised angiography of brain for operative planning. The procedure could have become easier had Digital Subtraction Angiography (DSA) would have available. It is a fluoroscopy technique used in interventional radiology to clearly visualise blood vessels in a bony or dense soft tissue environment. The DSA facility is available at Trauma Centre. But Trauma Centre where DSA facility is available is converted into COVID Hospital. This has compelled team of doctors to conduct the procedure at Super.
 
Dr Pramod Giri took decision to post patient for surgery on the basis of CT angiography Brain only. Patient underwent surgery at Neurosurgery Department at Super Speciality Hospital. Dr Pramod Giri and his team Dr Sagar Shahane and Dr Sanjog Gajbhiye carried out complex surgery, where the innovative approach of endoscope assisted clipping of aneurysm carried out. Dr Sajal Mitra, Dean of GMCH, motivated the emergency management of this case by providing all the support. The surgery proved beneficial to patient as she has improved post-operatively and doing well in ward. So, apart from difficulties faced by healthcare workers because of COVID-19 and lockdown, more than 25 neurosurgical procedures carried out at department of SSH Nagpur. The Present Case is one of the rare types i.e Telovillo Tonsillar branch of medial trunk of posterior inferior and cerebellar artery bilobed aneurysm. Hardly 8-10 cases of this particular aneurysm were noted in medical science literature in the world.
