The State Cancer Institute (SCI) in Jabalpur has reached a significant clinical milestone, reinforcing its position as a premier center for advanced oncological care in Central India. By successfully performing a complex breast reconstruction following a nipple-sparing mastectomy or Breast Implant Replacement, the institute has demonstrated a sophisticated integration of surgical oncology and plastic surgery. This specific procedure represents a ‘first’ among state-run medical colleges in both Madhya Pradesh and broader Central India region. The surgical team at SCI executed a nipple-sparing mastectomy with immediate implant-based reconstruction for a young, unmarried patient diagnosed with multicentric breast cancer.
 
Unlike traditional radical mastectomies, this approach prioritises preservation of anatomical envelope and nipple-areola complex, which is critical for the patient’s psychological recovery and physical symmetry. Complexity of the case was compounded by the patient’s clinical history, which included skin loss resulting from prior lumpectomies performed at external facilities. To ensure a successful outcome, the surgical team including Professor Pawan Agrawal, Professor Prashant Yadav and Professor Rajeev Kukrele employed a Latissimus Dorsi (LD) Myocutaneous Flap to provide adequate soft tissue coverage for the implant.
 
This achievement was made possible through multidisciplinary collaboration of several senior specialists and departments like Surgical Oncology, Radiation Oncology, Plastic Surgery and General Surgery. Dean Dr Navneet Saxena and Superintendent Dr Arvind Sharma rendered administrative support. Dr Arpan Mishra, Oncologist at SCI, told “We at State Cancer Institute, Jabalpur, are constantly pushing boundaries of breast cancer treatment”. Dr Mishra informed that the patient, from a rural and underprivileged background, was highly motivated to undergo reconstructive surgery, driving our commitment to this complex procedure. To facilitate this, the ‘Breast Cancer Care Group’—a mentorship initiative composed of cancer survivors—stepped in to provide both emotional guidance and financial resources. This model of peer-led support combined with surgical excellence sets a new standard for public health institutions nationwide.
